ルⅣb）。Borderline resectable膵癌に対する術前化学療法単独での報告は少ないが，術 前 化 学 療 法 単 独 で も borderline resectable 症 例 の 予 後 が 改 善 す る 可 能 性 が あ る。
Borderline resectable症例33例中13例に手術を施行し，R0率は69％，生存期間中央値 は22カ月であった 10）（レベルⅣa）。
門脈・上腸間膜静脈浸潤症例109例のうち74例に術前化学放射線療法を施行したとこ ろ，全周性に狭窄している症例では術前化学放射線療法により予後が改善しなかった が，片側性の狭窄症例では術前化学放射線療法により生存期間は改善した（OS中央値：
CRTあり 26カ月 vs なし 10カ月，p＜0.0001） 3）（レベルⅣb）。
腹腔動脈浸潤を伴う膵癌は『NCCNガイドライン』（2012年版）では切除不能膵癌とし て扱われるが，腹腔動脈周囲への癌浸潤を伴う膵体部癌では腹腔動脈幹を合併切除する 膵尾側切除術が行われることがある。癌遺残のないR0切除率は37〜91％ 11─14），術後の 生存期間中央値は10〜21カ月 11─17）であった 14）（レベルⅣa） 11─13, 15─17）（レベルⅥ）。さらに 術前化学放射線療法施行後に腹腔動脈合併切除を伴う膵体尾部切除を施行することで R0率が91％，生存期間中央値が26カ月という報告があり 18）（レベルⅥ），腹腔動脈合併 切除することで局所制御効果が得られる症例もあることから，腹腔動脈合併切除にて切 除可能な膵癌をborderline resectable膵癌とみなすことが可能かはさらなる症例の蓄積 と検討が必要である。
■明日への提言
有効な術前治療の開発により術後の補助療法も含めた集学的治療を施行することで，
borderline resectable膵癌の予後は改善する可能性がある。さらに術前治療後に切除す ると生命予後延長効果が期待できる症例を選別することで，borderline resectable膵癌 の新たな治療方針を確立できる可能性がある。
