This questionnaire enables us to get an overview of the programme and how it is delivered. There is also an opportunity for you to provide us with any additional information you feel we need to know about.

We invite you to complete the questionnaire, providing full answers to each question in the boxes on this form. Please state clearly if any part of a question is not applicable. Your answers should provide information directly relating to the programme. If you reference any document in your answers, please indicate clearly the page/section of the attachment where the relevant information is found. Should any questions be unclear, please contact us at [email protected].

You will also need to complete the two annexes at the end of this questionnaire:

You should collate the documentation that you have listed as evidence in Annex One. All evidence that is available electronically, but does not contain confidential information relating to individuals (patients, students and staff) should be submitted to the GDC.

The completed form, annexes and documentary evidence should be uploaded to the GDC extranet page. Instructions for this will follow shortly.

The evidence that contains confidential and sensitive information should be available during the inspection in the meeting room allocated to the inspection panel. This can be in electronic or hard copy.
