Thrombomodulin protects againstlung damage created by highlevelof oxygen with largetidalvolumelneChanicalventilationinrats

BACKGROUND:
Ventilator−inducedlunginjury(VILl)is associated withinflammatory responsesin the lung・Thrombomodulin(TM),a COmpOnent Ofthe coagulation system,has anticoagulant and anti−inflammatory effects・We hypothesized that the administration of recombinant hulnanSOlubleTM(rhsTM)wouldblockthedevelopmentoflunginjury.
METIIODS:
LunglnJury WaSinduced by high tidalvolume ventilation for2h withlOO%oxygenin
rats・Rats were ventilated with a tidalvolume of35 ml/kg with pretreatmentvia a Subcutaneousinjection of 3 mg/kg rhsTM(HV(high tidalvolume)/TM)or saline
(HV/SAL)12h before mechanicalventilation.Rats ventilated with a tidalvolume of6 ml/kg underlOO%oxygen with rhsTM(LV(low tidalvolume)/TM)or saline(LV/SAL)
were used as controls. Lung protein permeability was deterlnined by Evans blue dye
(EBD)extravasation.
RESULTS:
LunglnJury WaS SuCCeSSfullyinducedin the HV/SAL group compared with the LV/SAL group,aS Shownby thesignificant decreaseinarterialoxygenpressure(PaO2),increased PrOteinpermeability,andincreaseinmeanpulmonaryarterypressure(mPAP)andratio of
meanpulmonary arterypressure to mean systemic arterialpressure(Pp/Ps).Treatment of
rats withlunginjurywithrhsTM(HV/TM)significantly attenuated the decreasein PaO2 and theincreasein both mPAP and Pp/Ps,Which was associated with a decreasein the lung protein permeability.Lung tissue mRNA expressions ofinterleukin(IL)−1α,IL−1β,
IL−6,tumOr neCrOSis factor−α,and macrophageinflammatory protein(MIP)−2 were Significantly higherin HV thanin LV rats・Rats with Vlu treated withrhsTM(HV/TM)
had significantlylower mRNA expressions ofIL−1cL,IL−1β,IL−6,and MIP−2than rats treatedwithsaline(HV/SAL).
CONCl.USIONS:
Administration ofrhsTM may prevent the development oflunglnJury Created by high levelof oxygen withlarge tidalvolume mechanicalventilation,Which has concomitant decreaseinproinflammatorycytokineandchemokineexpressioninthelung.
以上、本論文は、ラットにおける高濃度酸素下での高一回換気量による人工呼吸 誘発肺傷害の発生に対する TM による予防効果を示した論文であり、学術上極めて 有益であり、学位論文として価値あるものと認めた。
