4 ロイコボリン ® レスキュー（ロイコボリン ® 救済療法）
重篤な副作用発現時（重篤あるいは症状を伴う血球減少症など）には MTX を中 止するとともに，活性型葉酸製剤であるロイコボリン
®
投与を行う．ロイコボリ ン®
錠 10 mg，6 時間ごとに経口投与 ，あるいはロイコボリン®
注 6 〜 12 mg，6 時間ごと筋注あるいは静注投与する（ロイコボリン
®
の 1 日投与量は MTX 投与 量の 3 倍程度を目安とする．MTX 8 mg/ 週投与中であればロイコボリン®
は24 mg/ 日程度）．MTX の排泄を促す目的で十分な輸液と尿のアルカリ化を行う．
ロイコボリン
®
投与は副作用が改善するまで行う．MTX による血球減少症に対し てロイコボリン®
救済療法を行った症例を提示する（図 4）．■図 4 白血球減少症と発現時の処置（56 歳女性，罹病歴 12 年）
MTX服用歴６年．MTX 10 mg/週，葉酸5 mg/週で経過良好であったが，下痢，腹痛に伴い経口摂取不良となった．
白血球減少（900/fL）が出現したため，MTX を中止し，G-CSF 投与とロイコボリン
®
レスキューを行い回復した．フォリアミン® 5 mg/ 週

第 5 章 投与開始前のスクリーニング● 53 MTX 投与開始前に，関節リウマチ（RA）活動性評価ならびに MTX の副作 用の危険因子の評価に必要な問診と診察，末梢血検査，赤沈，一般生化学 検査，免疫血清学的検査，胸部 X 線検査に加え，肝炎ウイルスと結核のス クリーニング検査を実施する．
MTX 投与開始前の検査として，禁忌や慎重投与の項目の有無，副作用の危険因 子など MTX 投与が適正か否かの判断，RA 活動性や予後の把握に必要な検査に加 えて，投与後の有効性や副作用のモニタリングに必要な検査項目もチェックする．
