Does Laparoscopic Gynecological Surgery Increase Postoperative Complications in Elderly Patients?

Abstract

Objective: As life expectancy increases, surgeons have more opportunities to treat elderly patients.

Laparoscopic surgery is minimally invasive in comparison to open surgery and is gradually replacing common surgical techniques.

We aimed to evaluate the usefulness and safety of laparoscopic gynecological surgery in elderly patients.

Methods: In this retrospective analysis, we investigated the demographics, postoperative outcomes and complications of 178 patients with benign ovarian or uterine tumor, who were ≧ 60 years of age, and who underwent salpingo-oophorectomy or hysterectomy from January 2011 to December 2017 at Ehime Prefectural Central Hospital.

We compared the BMI, operative time, estimated bleeding, and Charlson comorbidity index values of patients undergoing laparoscopy and laparotomy. Various statistical analyses, including t-tests, Fisher’s exact test, the Mann-Whitney U test, Spearman’s correlation coefficient by rank test, and logistic regression analyses, were performed to analyze the results. P values of <0.05 were considered to indicate statistical significance.

Results: Postoperative complications tended to increase in patients who were ≧ 80 years old or more. Patients who underwent laparoscopic surgery had fewer complications than those who underwent laparotomy. Patients with a CCI score of ≧ -3 had a significantly higher rate of postoperative complications than those with a score of 0-2 (hazard ratio: 7.2; p<0.05) .

Conclusion: The results of the present study demonstrated that laparoscopic surgery is useful and safe for elderly patients.
