Amiodaroneの投与法 1)初期急速投与
アンカロン注○R 2.5ml (=125mg) + 5% glucose 100ml 600ml/hr
2)負荷投与
アンカロン注○R 15ml (=750mg) + 5% glucose 500ml 33ml/hr 6時間
3)維持投与
アンカロン注○R 15ml (=750mg) + 5% glucose 500ml (負荷投与と同様の組成)
17ml/hr
4)点滴静注から内服への移行(当科プロトコール)
1)-3)の点滴静注を計4日間で終了し、5日目からアンカロン○R200mg/dayを維持量として開始
ただし消化吸収不良が疑われれば別に対応。
*12誘導心電図モニター(投与開始直後、6時間後、24時間後。以後一日一回 14日目まで)
**amiodarone血中濃度を内服開始の朝と内服開始後14日目の朝にとる。
***TdP、徐脈、その他の副作用(肺線維症、甲状腺機能、肝機能)をフォロー
