Aneurysm Volume‐to‐Ostium Area Ratio (VOR); A Parameter Useful in Discriminating the Rupture Status of Intracranial Aneurysms

Background and Purpose: Slow flow is one hemodynamic feature that has been linked to the risk of aneurysm rupture. It was our objective to assess the potential value of the ratio of the aneurysm volume to its ostium area (VOR) as an indicator of intra‐aneurismal slow flow, and thus, in turn, the risk of rupture.
Materials and Methods: One hundred‐fifty five consecutive aneurysms were studied retrospectively. Measurements of aneurysm size, aspect ratio (AR) and VOR were performed using commercial software. These parameters were correlated with rupture status (ruptured or unruptured). A multiple logistic regression model that best correlated with rupture status was generated and then validated prospectively using an independent database of 62 consecutive aneurysms acquired outside of the retrospective study interval.
Results: VOR showed the best discrimination for rupture status among the parameters. The best logistic regression model, which contained VOR rather than size or AR, discriminated rupture status correctly in 80.6% of the prospective cohort. The reproducibility calculating AR and VOR was excellent.
Conclusions: Determination of VOR was easily and reproducible done using widely available commercial equipment. It may be a more robust predictor of aneurysm rupture than is AR.
Further assessment in larger prospective studies is warranted.
