●活動内容
1 要支援 1・2 の方に対する介護予防サービス利用 のプラン作成
2 支援や介護が必要となる恐れが高い方への介護予 防ケアプランの作成
3 認知症予防や転倒予防などの介護予防教室の開催 4 介護をしている家族の方向けの教室や交流会の開催 5 高齢者やその家族に対する相談窓口の開設 6 民生委員や介護支援専門員（ケアマネ）など、高
齢者に関わる人とのネットワークづくり 7 成年後見制度の紹介
8 高齢者虐待の早期発見
9 悪質商法の被害防止のための教室の開催 10 高齢者への戸別訪問

Welfare for the Elderly
■ Inclusive Community Support Centers
● Purposes
1. Prevent elderly people from development or deterioration of conditions requiring nursing care, or alleviate such conditions;
2. Provide comprehensive support to elderly people and their families;
3. Support those engaged in nursing care in creating a well-developed care system; and
4. Help promote the rights of the elderly, including abuse prevention.
● Scope of activities
1. Developing care preventive service plans for the elderly in care needs levels 1 and 2
2. Developing care preventive service plans for those at a high risk of becoming in need of support or nursing care 3. Holding classes/seminars for elderly people about prevention of cognitive impairment, fall prevention measures, etc.
4. Holding classes/seminars and information exchange events for care-giving families
5. Providing counseling services to the elderly and their families
6. Developing networks with those working for nursing care, such as welfare commissioners and nursing care managers 7. Giving an introduction to adult guardianship
8. Detecting elderly abuse in the early stage
9. Holding classes/seminars to prevent elderly people from becoming victims of illegal business
10. Visiting elderly persons in their homes
