How to fill in “Application for Extension of Period of Stay” 【Meiji University】

申請書を提出する地方入国管理局を記入してください。支 局・出張所の場合は管轄する地方入国管理局を記入してく ださい。
Please write the name of Regional Office which you apply to. If you apply to the Branch Office or the District Immigration Office, please fill out the name of Regional Office which have jurisdiction over the Branch Office or the District Immigration Office.
申請人が有する国籍を記入してください。台湾，香港，マカオ の場合は台湾，中国（香港），中国（マカオ）と記入してくださ い。
Please write your country name. If you have Taiwanese or Hong Kong Nationality ,please write "Taiwan" ,"China (Hong Kong)" or "China (Macau)".
添付する写真の裏面には，必ず氏名を記入してください。
Please make sure to write your name on the back side of your photo.
英字氏名を姓，名の順に記入してください。
Please write your family name in English first, and write your given name in English.
日本に住む親族や同居家族がいる場合は，その名前等を記入し て下さい。いない場合は「なし」と記入してください。
If you have any relative in Japan or you live in Japan with your family, please write their names etc.. If you don't have, please leave them blank.
どちらかを○で囲んでください。
犯罪（日本国外のものも含む）を理由とする処分を受けた ことがある場合には，罪名及び処分内容等を具体的に記入 してください。
Please make sure to circle "Yes" or "No".
If you have a criminal record or deportation record, please write the detail concretely.
母国の住所を記入してください。中国・べトナム国籍者 は〇〇省〇〇市（県）～号まで記載してください。
Please write your home town address in alphabet. Chinese and Vietnamese are required to write all numbers including street number (all dijits).
13番：卒業までに必要な年月数を記入してください。
Please write as number of years and months until graduation.
（例）
卒業までに3年かかる場合→3年 卒業までに1年8ヶ月かかる場合→1.8年 《Example》

※申請者本人の貯蓄から支出する場合

*In case you support by yourself・「(1)支弁方法及び月平均支弁額」で「本人負担」を選 択し，月々使用する金額を記入。
If you financially support yourself fully, please select "Self" of "(1)Method of support and amount of supporter per month" and write the amount per month.

If your supporter lives abroad, please select "Supporter living abroad",and write the amount per month.
・母国から送金する場合，「(2) 送金・携行等の別」で「外国か らの送金」を選択し，月々の金額を記入。
If you receive the money from overseas every month, please select "Remittances from abroad" and write the amount per month.
・外国から携行する場合，「(2) 送金・携行等の別」で「外国か らの携行」を選択し，金額等を記入。
If the other persons ( your parents etc. ) bring the money for you from overseas to Japan, please select "Carrying from abroad" and write the amount.
・「(3)経費支弁者」に経費支弁者情報を記入。
Please write supporter's information to"(3) Supporter".
新入生は最終学歴情報を記入してください。 なお，最終校には日本語学校は含みません。 明治大学在学生は「在学中」を選択してください。 《For incoming students》

Please select "In school" and write information about Meiji University.

"２６ （１）支弁方法及び月平均支弁額"で奨学金を選択した 場合，奨学金の支給機関を選択・記入してください。
《例》日本学生支援機構からの奨学金の支給が決定してい る場合は，"公益社団法人又は公益財団法人"を選択し， ( )内に「JASSO」と記入してください。
If you select 'Scholarship' in "26 (1) Method of support to meet the expense while in Japan" , please select the scholarship organization and write its name.
《Example》 If you decided to receive the scholarship from
Japan Student Services Organization, please select 'Public interest incorporated association or public interest incorporated foundation' here and fill out ' JASSO' in the parenthesis.
署名の上，申請書を書いた日付を記入してください。
Please write your signature and the date of filling in this form.
どちらかを○で囲んでください。資格外活動をしている場合は， その内容を具体的に記入してください。

申請人本人と"(3)経費支弁者"との関係を選択してく ださい。本人負担の場合は記入不要です。

"20 hours" ( 12 classes × 100 min÷60min).
所属する学部・課程を選択してください。
Please select your faculty or course.
情報コミュニケーション学・国際日本学・教養デザイン研究科・ ガバナンス研究科・グローバルビジネス研究科・会計専門職研究科の 場合は，"その他人文・社会科学"を選択し，括弧内に「情報コミュニ ケーション学」あるいは「国際日本学」，「教養デザイン」，「ガバナンス」， 「グローバルビジネス」，「会計専門職」と記入してください。
If you'll be in the School of Information and Communication, the School of Global Japanese Studies, the Graduate School of Humanities, the Graduate School of Governance, the Graduate School of Global Business or the Graduate School of Professional Accountancy, please select " Others(cultural science / social science)" and write 'Information and Communication', 'Global Japanese Studies', 'Humanities', 'Governance', 'Global Business' or ' Professional Accountancy' in the parenthesis.
また，理工学・総合数理学・先端数理科学の場合は，"その他自然科 学"を選択し，括弧内に「理工学」あるいは「総合数理学」，「先端数理 科学」と記入してください。
大学院生は所属研究室名・指導教員名を記入してください。
If you are Doctor, Master or Graduate school (Research student), please write the name of your laboratory and mentoring professor.
研究室名がない場合には，研究分野名を記入してください。
If your laboratory doesn't have a name, please fill in your research field.
卒業予定年月を記入してください。
Please fill out your scheduled period of education until graduation.
