How Does It Work?

Foreign residents living in Japan for more than three months and not covered by the Employees’ Health Insurance Plan are required to join the National Health Insurance Plan in Katsushika.

In some cases, even those who stay in Japan less than three months may be eligible to join the National Health Insurance Plan. Contact the Eligibility Division for details.

The following table shows the ratio of your payment of medical expenses when you become a member of the health insurance plan.

If you meet the eligibility requirements for the National Health Insurance and agree to the following cases, register for the entry within 14 days.

① Entered Japan and got the resident registration in Katsushika. ② When moving in to Katsushika City from another municipality. ③ When a baby is born.

④ When you withdraw from the Employees’ Health Insurance Plan. ⑤ When you withdraw from the social welfare benefits.

!!Attention!!

● If the registration procedure is delayed, you need to enter the plan retroactively to the date of your entrance to Japan, moving in to Katsushika or loss of the qualification up to two years. The premium will also be claimed retroactively.

● You need to pay the full amount of medical expenses until the entrance procedure is completed. You will not be refunded. ● The presence of your admission to the National Health Insurance

Plan may influences your visa renewal.

Procedure

Report within 14 days when changes of the registered items took place, such cases as ①moving your domicile, ②the head of the household or your name changed, ③moving your household, ④your family member changed, ⑤joined the Employees’ Health Insurance Plan and ⑥joined the social welfare benefits.

※ 転 出てんしゅつ・出 国しゅっこく及びおよ 死亡し ぼ うしたときは、葛飾区か つ し か くの保険証ほけんしょうは使つかえなくなりますので、 国保こ く ほ年金課ね ん き ん か資格係しかくがかりへお返かえしください。 ※保険証ほけんしょうをなくしたとき、または汚よごれて使つかえなくなったときも、本人ほんにんであるこ とを確認かくにんできるもの（パスポート・在 留ざいりゅうカードまたは特別とくべつえいじゅうしゃ永 住 者しょうめいしょ証 明 書な ど）と、個人こ じ ん番号ばんごうを確認かくにんできるもの（個人こ じ ん番号ばんごうカードまたは通知つ う ちカード）を持参じ さ ん の上うえ、再交付さ い こ う ふの申請しんせいをしてください。 保険料 ほけんりょう ４月がつから翌年よくねん３月がつまでの１年間分ねんかんぶんの保険料ほけんりょうは、年齢ねんれいや人数にんずう、前年ぜんねんの所得しょとくなど を基もとにして、世帯せ た い単位た ん いで計算けいさんしますので、世帯せ た いの 状 況じょうきょうにより異ことなります。 保険料 ほけんりょう は国民こくみん健康けんこう保険ほ け んに加入かにゅうしているすべての方かたにかかる「医療分いりょうぶん保険料ほけんりょう」 と「支援し え ん金分きんぶん保険料ほけんりょう」、国民こくみん健康けんこう保険ほ け んに加入かにゅうしている方かたのうち４０歳さいから６４歳さい までの方かたにかかる「介護分か い ご ぶ ん保険料ほけんりょう」の合計ごうけい額がくとなります。 なお、世帯せ た い主ぬしが会社かいしゃの健康けんこう保険ほ け んなどに加入かにゅうしている場合ば あ いでも、世帯せ た い主ぬしに保険料ほけんりょう を納おさめる義務ぎ むがあります。くわしくは、国保こ く ほ年金課ね ん き ん か資格係しかくがかりへお問とい合あわせくだ さい。 ※国保こ く ほに加入かにゅうしている方かたは、所得しょとくがない場合ば あ いでも 住 民 税じゅうみんぜいの申告しんこくが必要ひつようです。 申告 しんこく がないと、保険料ほけんりょうの計算けいさんを正ただしく 行おこなうことができないだけでなく、高額こうがく 療養費 りょうようひ の 払 戻はらいもどし（P.44）や 入 院にゅういん時じ食事しょくじ療養費りょうようひの減額げんがく（P.44）等とうが受うけられ なくなる場合ば あ いがあります。住 民 税じゅうみんぜいの申告しんこくをしていない方かたは、保険料ほけんりょう決定けってい（変更へんこう） 通知書つ う ち し ょの〔３保険料ほけんりょうの個人こ じ ん別べつうちわけ内訳と加入かにゅう期間き か んは、次つぎのとおりです。〕の「所得しょとく 割額 わりがく の算さんしゅつ出基礎き そ」に「所得しょとくじょうほう情 報不明ふ め い」と記載き さ いされています。 ※住 民 税じゅうみんぜいについては、１２８ページ、１３０ページをご覧らんください。

* Return the National Health Insurance Card issued by Katsushika City to the National Health Insurance Section when you move out of Katsushika City or leave Japan or when a card holder has passed away.

* In case you lost the insurance card or it was damaged, apply for the reissue with a verification ID (passport, resident card or special permanent resident certificate) and your individual number (individual number card or notification card).

Insurance Premium

The insurance premium of a year starting from April to next March is calculated based on the ages and number of family members, income during the previous year, etc., per household.

Insurance premium is the total amount of “the portion for medical treatment” and “the portion for support”; both are imposed on all the member of the National Health Insurance Plan, as well as “the portion for Long-term Care” imposed on those who are between 40 and 64 of ages.

The head of household is responsible for the payment of the premium even if he/she is a member of other health plan. For details, contact the Eligibility Section, National Health Insurance & Pension Section. ※All the member of the National Health Insurance Plan needs to file a

residence tax even if there is no income. Otherwise, your insurance premium will not be properly calculated, and you may not be able to receive benefits such as “the reimbursement of large medical

expenses (p.45)”, “the reduction of meals and medical treatment expenses during hospitalization (P.45)” etc. For those who have not filed a residence tax, “Basis of Calculation for Income Based Levy” is marked as “Income Information Unknown” in section 3 [Insurance Premium per Individual and Insured Period] of the Notification of Insurance Premium.

The premiums can be paid through account transfer or by cash at banks, Japan Post Bank, post offices, convenience stores, National Health Insurance and Pension Section at Katsushika City Office or branch offices with the statement of payment mailed by the City Office. Application for account transfer is available at banks in Katsushika, Japan Post Bank, post offices and National Health Insurance and Pension Section. Contact the Collection Division for details.

What Happens if Premium cannot be Paid?

Default without special reasons may impose you a full payment of medical expense at medical institutions and your property may be seized in some cases.

Please contact the National Health Insurance section for a payment plan which is best suitable for your circumstances.

Reimbursement of Medical Expenses

In the following cases, you can apply for the reimbursement of your medical expenses at a set rate pending city approval.

① When you were treated without the insurance card (for emergency only). ② When you received massages, acupuncture or moxibustion treatment with

your doctor’s instruction. And when you received judo therapist’s treatment and made the payment by yourself.

③ The expense of garments, such as a corset, needed for medical treatment with doctor’s instruction.

④ Treated in overseas after falling suddenly ill or ingured. ⑤ Received blood transfusion.

⑥ Transportation fee for patients of serious condition with doctor’s instruction and approval by the City.

高額 こうがく 療養費 りょうようひ の払い戻はら もどし 医療いりょう機関き か ん等とうに支払ったし は ら 医療費い り ょ う ひの一部い ち ぶ負担ふ た ん金きんが一定いっていの金額きんがくを超こえたときに、そ の超こえた金額きんがくが払い戻はら もどされます。 限度げ ん ど額がく適用てきようにんていしょう認 定 証 医療 いりょう 機関き か ん等とうに支払し は らう医療費い り ょ う ひの一部い ち ぶ負担ふ た ん金きんについて、事前じ ぜ んに申請しんせいすることで 一定 いってい の金額きんがくを 上 限じょうげんとする支払しはらいで済すむようになります。交付こ う ふには 条 件じょうけんがありま す。 入 院 にゅういん 時じ食事しょくじ療養費りょうようひ 住 民 税 じゅうみんぜい が非課税ひ か ぜ い世帯せ た いの方かたは、事前じ ぜ んに申請しんせいすることで食事しょくじ療養費りょうようひが減額げんがくされます。 出 産 しゅっさん 育児い く じ一時い ち じ金きん 加入者 かにゅうしゃ が 出 産しゅっさんしたとき、赤あかちゃん１ひと人りにつき４２万円まんえんが支給しきゅうされます。日本に ほ ん 国内 こくない での 出 産しゅっさんに限かぎり、分娩ぶんべん費用ひ よ うとして 直 接ちょくせつ医療いりょう機関き か んに支払し は らうこともできます。 葬祭費そ う さ い ひ 加入者 かにゅうしゃ が死亡し ぼ うしたとき、葬儀そ う ぎを 行おこない葬儀そ う ぎ費用ひ よ うを支払し は らった人ひとに７万円まんえんが支給しきゅうさ れます。

Reimbursement of Large Medical Expenses

If you paid more than a designated amount in medical expenses, the excess will be reimbursed.

Maximum Ceiling Amount Applicable Certificate

With a prior application, you might be only required to pay a pre-fixed ceiling amount at the cashers of medical facility etc. Conditions apply.

Meals and Medical Treatment Expenses during Hospitalization

The reduction of the meals and medical treatment fees for a member of the resident tax exempted family is available with a prior application.

Lump-sum Childbirth/ Childcare Subsidy

Upon childbirth by the insured, the subsidy of \420,000 per child is paid. It is applicable only for the birth in Japan, and it can be paid directly to the medical facility to cover the delivery expenses.

Lump-sum Funeral Expense Subsidy

Upon the death of the insured, \70,000 will be paid to the person who made the payment for the funeral arrangement.

All who live in Japan between 20 to 59 years old are required to enroll in the National Pension Plan. Bring your passport or the residence card to the above mentioned office to register. Those who work for company or factory that enrolls you into an Employees’ Pension Plan do not need to register.

The statement of payment, collected from the month of your enrollment, will be delivered to you by Japan Pension Service. The payment can be made at financial institutions such as post office, bank and convenience store, etc.

If you and your family have a financial difficulty to make the payment, you can apply for the payment exemption, the students’ exceptional payment arrangement and the grace of payment. These arrangements could be made depend on your financial state such as the amount of income made by you/ spouse/ head of household. Please contact for details.

To receive a pension, you need to have paid the set premium for more than 10 years.

Foreigners who have paid into the National Pension Plan for 6 months and more are entitled to receive a lump sum pension withdrawal benefit provided the application be made within 2 years of the departure. For details, contact Katsushika Pension Office.

What is Long-term Care Insurance Plan?

The Long-term Care Insurance Plan is a system which society as a whole supports the long-term nursing care of the elderly to ease their anxieties and to provide comfortable life in their hometown.

In order to use the long-term care service, you need to obtain the certification of needed long-term care. For the Long-term Care Insurance Plan, please contact the Long-term Care Insurance Section, Administration Section for the details.

Those Covered under Long-term Care Insurance Those who have address in Katsushika City and

① Residents who are members of a medical insurance program and between above 40 and under 65 of ages, or

② Residents aged 65 and older.

※ A Long-term Care Insurance identification certificate is sent to those who are applicable to ① who are approved as a long-term Care requirer, and all who are applicable to ②.

※ Please return your long-term care insurance identification certificate to the Long-term Care Insurance Section Qualification Receiver Division when you are moving out of Katsushika City, leaving Japan or when deceased as the certificate issued by Katsushika City will be invalid.

Premiums

Those who are under 64 years of age must pay the premiums for Long-term Care Insurance together with medical insurance premiums (health insurance premiums). The premiums for Long-term Care Insurance are determined by each medical insurance plan. Contact your medical insurance office for details.

For 65 years of age and over can pay the premium by automatic deduction from National Pension, through the statement of payment issued by the city or through account transfer. For details, contact the Long-term Care Insurance Section Qualification Receiver Division.
