目的;近年, 慢性腎臓病(Chronic Kidney Disease; CKD)の高い
有病率が大きな関心を集めている. 高尿酸血症, 生活習慣要
因を含めた CKD のリスク要因の横断的検討を目的とした.
方法;2007 年 1 月~12 月に都内健診センターで人間ドック
を受診した 30 才代~50 才代の 9499 名のうち, 腎疾患の既
往を持つ 122 名を除外した 9377 名(男性 6144 名, 女性 3233
名)を分析対象とした. 高血圧症, 高尿酸血症, 糖尿病, 脂
質異常症と生活習慣要因について多重ロジスティック回帰
分析を用いて CKD との関連を検討した.
結果;CKD の有病率は男性 10.6%, 女性 8.7%であった.
男女ともに高血圧症(オッズ比;男性 2.17, 女性 1.78), 高
尿酸血症(オッズ比; 男性 1.78, 女性 3.06)が CKD と有意
に関連していた. 男性では血清尿酸値と CKD 有病率に正の
相関を認めた.
結語;血清尿酸値が男女ともに CKD の独立した危険因子で
あることが示唆された. このことから血清尿酸値の適切な
スクリーニングが CKD 予防に重要であると思われる.
3 英文 Abstracts
Background: An increase in the rates of chronic kidney disease (CKD) are being widely
observed all over the world, due to increases in hypertension, diabetes mellitus and
aging. Similarly, in Japan, the number of patients undergoing dialysis nearly doubled in
the 15 years to 2010, reaching 297,000, and raising concerns about the increased risk
of cardiovascular events and related socio-economic issues. In addition, patient
populations with multiple atherosclerotic risk factors have also increased with a
corresponding number of growing elderly patients. While prevention of kidney
nephropathy and early detection of CKD are important, large-scale studies on the
prevalence of CKD in Japan are limited.
Objectives: To explore the prevalence and risk factors of CKD, including hyperuricemia,
and life style factors in a sample drawn from the Japanese health screening population.
Methods: A cross-sectional study was conducted to assess the prevalence and risk
factors of CKD. Study subjects aged 30 to 59 years totaled 9499 individuals who
participated in the annual health screening program in the Tokyo metropolitan area from
January 2007 to December 2007. 122 individuals with a history of renal disease were
excluded. CKD was diagnosed by an estimated glomelular filtration ratio (eGFR) of less

Nephrology. Univariate and multivariate logistic regression analysis were performed to
evaluate association with CKD risk factors.
Results: Mean age of the 9,377 subjects was 46.0 years for men, and 45.3 years for
women. 6,144 (65.5%) were men, and the prevalence of CKD was 10.6 % in men, and
8.7 % in women. Hypertension {Odds Ratio (OR) = 2.17, 95% Confidence Interval
(95%CI) = 1.77-2.56 in men and OR = 1.78, 95%CI = 1.15-2.68 in women} and
hyperuricemia(OR = 1.78, 95%CI = 1.48-2.14 in men, and OR = 3.06, 95%CI =
2.01-4.56 in women) were found to have a significant association with CKD in both
men and women. There was a positive correlation in the men to serum uric acid
concentration and prevalence of CKD. These results were confirmed in previous
studies.
Conclusion: Hyperuricemia, and hypertension seemed to be the major risk factors for
CKD as per logistic regression analysis among female as well as male participants. Our
findings suggest that appropriate screening of uric acid levels, early detection of
microalbuminuria, promotion of healthy lifestyles, and early intervention with drug
therapy for hyperuricemia or hypertension are necessary for the prevention of CKD in

Ⅰ 背景
様 々 な 疫 学 調 査 か ら 日 本 に お け る 慢 性 腎 臓 病
( Chronic Kidney Disease; CKD) の実態が明らかと
なり、CKD の高い有病率が注目を集めている.
日本腎臓病学会の CKD 診療ガイド 2012 によると,
40 歳~74 歳の 2008 年度特定健診受診者で eGFR
(estimated glomerular filtration rate:推算糸球
体濾過量)が 60ml/min/1.73m
2未満のものは 14.5%
と し て い る 1). ま た , Imai ら に よ る と eGFR
60ml/min/1.73m
2未満のものは 20 才以上の約 13%に
あたる 1330 万人と推定されている 2). 更に 2010 年
度の日本における透析患者数は約 30 万人で, 15 年前
と比較して約 2 倍に増加している 3). 日本の CKD 増
加の要因としては, 高血圧や糖尿病といった生活習
慣病の増加に伴う腎硬化症や糖尿病性腎症の増加,
高齢化などが指摘されている.
CKD はその進行に伴い, 末期腎不全(ESRD)お
よび心血管系疾患(CVD)の重要な危険因子になり

うることが報告されており 4-5), それらの合併症は
患者個人の QOL の低下を来すとともに, その治療に
伴う医療費の増加は, 社会保障費が増加傾向にある
日本において重要な課題であり適切な対処が求めら
れている 6)
. また, 世界的にも CKD の有病率は増加
傾向にあり 7), 日本と同様に CKD は公衆衛生学的な
課題である 8)
. 一方, CKD の病期は eGFR 値によっ
て 1 期から 5 期に分類されるが, 1 期から 2 期の CKD
初期では自覚症状に乏しいため, スクリーニングに
よる早期発見が必要不可欠となるが, 一般住民にお
ける CKD の認知度は未だ低く, 基本健診の受診率も
低い状況にある.
CKD の危険因子については様々な検討がなされて
いるが, その中で高尿酸血症は既知の循環器疾患の
危険因子であるのと同様に, CKD 発症のリスク要因
であるという先行研究が報告されているが 9-13),
CKD と尿酸との関連を検討した研究は未だ限られて
おり, 特に日本人女性では報告がない. そして CKD

患者において高尿酸血症は CVD のリスクを増大させ
るが, CKD 増悪のリスクとはならないとの報告 14)も
あり, 更なる検討が求められている. 我々は受診率が
高い職場健診受診者を対象として CKD の関連要因を
検討してきた 15).
以上の背景より, 本研究では CKD のスクリーニン
グとしても優れている人間ドックに注目し, ①人間
ドック受診者の CKD の有病率とその特徴を明らかに
すること, ②従来の CKD の危険因子に血清尿酸値な
どを含めて, CKD の関連要因を幅広く横断的に検討
することを目的とした.
Ⅱ 対象および方法
1.対象
2007 年 1 月から 12 月に都内 Y 総合健診センター
にて半日人間ドックを受診した 30 才~59 才の 9499
名のうち, 腎疾患の既往歴および現病歴を持つ 122
名を除外した 9,377 名(男性 6,144 名, 女性 3,233 名)
を分析対象とした. Y 総合健診センターは都心部に立

地することから受診者として企業健診が多くを占め
ておりホワイトカラーが多い傾向にある.
2.方法
本研究は横断研究である. 調査項目は, 2007 年時
の 基 本 項 目 ( 年 齢 , 性 別 , 体 格 指 数 (Body Mass
Index;BMI), 治療歴;高尿酸血症, 高血圧, 糖尿病,
脂質異常症, 脳血管疾患, 心疾患), 検査データ(血
清クレアチニン値, 血清尿酸値, HbA1c(JDS), 空
腹時血糖値, 収縮期血圧, 拡張期血圧, 脂質関連項目,
尿蛋白), 自記式アンケートによる「ブレスローの7
つの健康習慣」に準じた生活習慣関連項目(喫煙, 飲
酒, 運動, 睡眠時間, 欠食, 間食)である 16). CKD の
診断については日本腎臓病学会による CKD の診断基
準を参考に「eGFR が 60 未満あるいは蛋白尿 1+以上
を認めるもの」と定義した. なお, eGFR は日本腎臓
病学会の日本人を対象とした GFR 換算式(eGFR
(mL/min/1.73 ㎡)= 194×Cr-
1.094×年齢-
0.287(女性は
×0.739))を用いて算出した.

各測定値および生活習慣の診断あるいは定義につい
ては, 肥満(BMI 25 以上), 高血圧(収縮期血圧
140mmHg 以上, または拡張期血圧 90 mmHg 以上,
あるいは高血圧治療中), 蛋白尿(尿蛋白定性1+以
上), 高尿酸血症(尿酸値が男性 7.0mg/dl 以上, 女性
6.0mg/dl 以上, あるいは高尿酸血症治療中), 糖尿病
(HbA1c 高値 6.1%(JDS)以上, または空腹時血糖
126mg/dl 以上,あるいは糖尿病治療中), 脂質異常症
(中性脂肪 150mg/dl 以上,または HDL コレステロー
ル 40mg/dl 未 満 , ま た は LDL コ レ ス テ ロ ー ル
140mg/dl 以上,あるいは脂質異常症治療中), 飲酒あ
り(毎日飲酒する), 喫煙あり(現在喫煙習慣がある),
間食あり(毎日間食する), 欠食あり(食事を抜くこ
とがある), 運動不足あり(定期的運動習慣がない),
睡眠不足あり(睡眠時間6時間未満)とした. 本研究
は順天堂大学倫理委員会の承認を得て実施した.
3.統計学的解析
CKD の関連要因については, 男女別に単変量解析

(ロジスティック回帰分析)で有意であった項目もし
くは関連傾向を認めた項目を多変量解析(多重ロジス
ティック回帰分析)に投入した. 多変量解析について
は生活習慣関連項目の投入の有無別に 2 つのモデル
を設定し CKD との関連を検討した(モデル1:測定
データのみ. モデル2:測定データおよび生活習慣関
連項目). 統計学的有意水準については p<0.05 を有
意とした. 統計解析には JMP ver.9(Windows 版)
を使用した.
Ⅲ 結果
平均年齢は男性 46.0±7.7 才, 女性 45.3±7.8 才で,
年齢分布は 30 才代から 50 才代の中若年層が中心で
あった. 肥満(BMI≧25)の割合は男性 29.2%, 女性
11.9%, 高血圧症は男性 20.6%, 女性 8.6%, 高尿酸
血症は男性 27.4%, 女性 5.2%,
糖尿病は男性 7.9%,
女性 2.8%, 脂質異常症は男性 48.6%, 女性 26.3%で
あった. また生活習慣関連項目については男女それ
ぞれ次の通りである;飲酒習慣あり(30.5%, 9.4%),

喫煙習慣あり(男性 38.4%, 女性 10.0%), 欠食あり
(34.0%,26.9%), 間食あり(16.2%,30.8%), 運動
不 足 あ り ( 39.6 % , 43.8 % ) , 睡 眠 不 足 あ り
(80.5%,78.1%)(Table 1.).CKD の有病率は男性
10.6%, 女性 8.7%で, CKD の有病率は男女ともに年

た(Table.2, 3). 一方, 男性では毎日飲酒が CKD と負
の関連を認めた. 男女ともに血清尿酸値と CKD の有

病率に正の関連を認めた.(Fig. 2)
Ⅳ 考察
近年 CKD への注目と相まって CKD の危険因子に
関する多くの研究が報告されているが, 高尿酸血症
と CKD との関連を調査した研究は限られている
9-14). Bellomo らによると献血者に対する前向きコ
ホート研究において, 高尿酸血症は男女ともに CKD
の独立した危険因子となることが報告されている 9).
また Obermayr らは高尿酸血症が CKD 新規発症の独
立した危険因子となることを報告している 11). 一
方日本では Kawashima らにより無症候性高尿酸血
症が男性で CKD のリスク要因となることを報告され
ているが 12),対象者数が少なく,解析が男性に限られ
るなど, 限界のある先行研究に限られている. 更に
男性では高尿酸血症と CKD の関連を示す先行研究が
いくつか見られるが, 女性では先行研究に乏しく, 本
研究は健診受診者を対象とした大規模な横断研究で
あり, 女性においても高尿酸血症が CKD の独立した

危険因子であることを証明した日本における初の報
告と思われる.
また今回, 男女ともに血清尿酸値の層別解析で
CKD 有病率に有意差を認め, 一般的には血清尿酸値
7mg/dl 以上で要経過観察とされるが, 血清尿酸値が
5mg/dl ないしは 6mg/dl を超えると CKD 有病率の
上昇を認めることから, 治療開始に関するカットオ
フ値の再考の必要性も示唆される. 実際に, Nakaya
らによると, アンケート調査ではあるが, 腎臓専門
医の約 85%がステージ 3 以上の CKD 患者における
無症候性高尿酸血症を CKD 進展予防および CVD 発
症予防のために治療すべきとの見解を示してしてお
り, 腎臓専門医の間では無症候性高尿酸血症を CKD
進展のリスクと認識している 17)ことからも, 健診
の際などに血清尿酸値の高値を認める場合には早期
介入の啓発が必要と思われる.
また高血圧, 高尿酸血症, 糖尿病, 脂質異常症など
の生活習慣病は, 健診などで指摘されても未受診率

が高いことも課題であり, 効果的な受診勧奨を行い
治療に繋げるために, 適切な受診指導が肝要と考え
られる. 更に既に治療中の生活習慣病を有する群に
おいて, 治療目標達成率が高血圧症で基礎疾患のな
い 65 歳以上で 68%との報告 18)もあり, 治療目標達
成率の向上も今後の課題の一つである.
本研究では生活習慣関連項目に関しても評価して
おり, 高血圧症, 糖尿病について考慮しても, 男性で
は間食が, 女性では欠食が CKD の独立した危険因子
であることが示唆された. 先行研究では, 食事回数が
減ると, 肥満につながるとされており, Matsushita
らは肥満者では蛋白尿の増加が見られるとの報告し
ている 19). 以上より, これらの生活習慣は直接的あ
るいは間接的に高血圧症や糖尿病, 高尿酸血症の発
症に関与することが予想され, 生活習慣に関しても
健診を活用したスクリーニングを行い, 包括的に生
活習慣に介入していくことが重要と考えられる.
本研究にはいくつかの限界がある. 第一は選択バ

イアスである. 本研究の研究対象は調査施設の立地
条件などから,いわゆるホワイトカラーが多い可能性
があり健康などに関して意識が高い集団の可能性が
ある. 更に健診センター受診者の多くは健保加入者
であり,30 才代から 50 才代の中若年層が中心である.
今後は複数地域および他施設での研究が必要とされ
る. 第二は情報バイアスである. 本研究では健診受診
時の自記式アンケートから生活習慣関連情報を収集
しており,過少申告されている可能性が否定できない.
飲酒についても飲酒習慣の評価に留まっており,量的
評価は出来ていないため,生活習慣関連項目のデータ
精度を高めるためにインタビューによる情報収集が
必要と思われる.
第三に空腹時採血ではない対象者が含まれる可能性
があることと,内服薬の詳細については追跡不能であ
ることが挙げられ,過少評価の可能性がある.最後に本
研究は横断研究であるため,CKD とリスク要因の因
果関係は言及できず,今後の縦断解析が必要である.

Ⅴ 結論
血清尿酸値高値が男女ともに CKD の独立した危険
因子であることが示唆された. このことから健診な
どを利用した血清尿酸値の適切なスクリーニングが
CKD 発症や進展予防のためには重要であると思われ
る.
