■母子保健
◎母子健康手帳の交付
妊娠していることがわかったら、市 役所、出張所、連絡所、証明センター、
総合保健センターのいずれかに妊娠届 出書を提出してください。母子健康手 帳(英語、ポルトガル語、タガログ語、
中国語、ハングル、インドネシア語、
スペイン語、タイ語を併記したものも あります)、母子健康手帳別冊、妊婦健 康診査受診票などを交付します。母子 健康手帳は妊産婦や乳幼児の健康状況 を記録するもので、健康診査や予防接 種を受ける際などに必要になります。
また、電子申請届け出サービスも利 用できます。（電子申請届け出先は市民 課のみ）
◎妊婦健康診査
市が委託している診療所・病院にお いて、診察・尿検査・貧血検査・血圧 測定、梅毒検査、B 型肝炎の検査、HIV 抗体検査(希望者のみ)、超音波検査など の費用の一部を助成しています。
◎産婦・新生児訪問指導
助産師などが、おおむね出産後2か 月までの希望者のお宅を訪問します。
育児のこと、産後の健康など、相談に 応じます。
◎こんにちは赤ちゃん事業 生後4か月までの乳児がいるご家庭 に、助産師などが訪問します。育児の ことや子育て支援に関する情報提供な ど相談に応じます。

■ Mother and child Health Programs
◎Issuance of a Mother-and-Child Health Handbook If you are expecting a child, please submit Pregnancy Report Sheet to either one of city hall, citizen registration branch offices, identification center or General Health Care Service Center. You will be issued a Mother-and-Child Health Handbook (Some editions include translation in English, Portuguese, Filipino, Chinese, Korean, Indonesian, Spanish and Thai), a Mother-and-Child Health Handbook Appendix and Pregnancy Health Examination Forms. The Mother-and-Child Handbook is a notebook for keeping a medical record for the expectant mother and infant. You must have this when receiving health examination and immunizations. An electronic application is available. (An electronic application is available for the Citizen Registration Division only.)
◎Examinations when Pregnant
Expectant mothers can take advantage of health checkups, urinalysis, anemia test, blood pressure measurement, one syphilis test, B-type hepatitis, HIV antibody test (Only those who so wish), ultrasound test by subsidizing part of the expense at clinics and hospitals that have contracts with the city.
◎Mothers after childbirth and Home Visit for Newborn Midwives make home visits to offer instruction for mothers generally for up to two months after childbirth. Please feel free to ask for advice on such topics as child-raising and healthcare after childbirth.
◎Hello new baby care program
Midwives make home visits to offer instruction for new baby within 4 months after birth. Please feel free to ask information on support of baby care and child raising, etc.
◎Infant and Toddler health Checkups
Health checkups are conducted for small children who are 4-months, 18 months and 39 months old at the General Health Care Service Center and Minami Public Hall.

■こども医療費の助成
こどもが医療機関などにかかったと きの、保険診療による一部負担金を助 成します。通院は小学3年生までのこ どもの医療費について、入院は 15 歳 に達する日以後、最初の3月末日まで のこども医療費についてが対象です。
小学 3年生までのこどもは、出生届、
転入届提出の際に登録申請書をお渡し しますので、必要事項を記入のうえ、
医療助成課（本庁舎2階）または出張 所・連絡所に提出してください（小学 4 年生以上は、事前の登録の必要はあ りません）。

■予防接種
予防接種には、総合保健センターな どで実施する集団接種（ポリオ）と、
委 託 医 療 機 関 で 実 施 す る 個 別 接 種
（BCG・三種混合・麻しん風しん混合 など）があります。詳細については「健 康づくりスケジュール」をご覧くださ い。同スケジュールは、毎年3月頃に 配布するほか、国民健康保険課（本庁 舎 2階）・市民課（本庁舎 1階）・総合 保健センター・出張所・連絡所で配布 します。
任意予防接種である子宮頸がん予防 ワクチン、ヒブワクチン、小児用肺炎 球菌ワクチンの接種費用に対して助成 を行っています。

■Subsidies for Medical Expense for children
Subsidies are available to alleviate part of the personally-borne medical expenses for children. In case of hospital visits, the child until the third year of elementary school is eligible and in case of hospitalization the child until the end of March after the child reaches 15 years of age. The application for registration is given to the child until the third grade of elementary school when submitting birth certification or move-in notification. Please fill in to submit it to the Medical Subsidy Division (2nd floor of the City Hall) or branch offices. It is unnecessary to register the child from the fourth grade up in advance.

■ Immunizations
Immunizations include group immunization (Poliomyelitis) conducted at locations such as General Health Care Service Center, etc., as well as individual immunization(BCG, DPT and Measles-rubella mixed) available at medical institutions that have contracts with the city. Please see the Health Building schedule for details. The detailed time schedule is distributed on/about March every year and is also available at the National Health Insurance Division (2nd floor of the City Hall), Citizen Registration Division (1st floor of the City Hall), General Health Care Service Center and Branch offices.
Subsidies are available for such optional immunization as prevention vaccine against cervical cancer, Hib vaccine and pneumococcal vaccine for child.

◎子ども手当
子ども手当とは次代の社会を担う子 どもの健やかな育ちを支援するための 手当です。中学修了前の子どもを養育 している方に支給されます。出生や転 入の際には、申請の手続きをしてくだ さい。
今後制度が変わることもあります。
◎児童扶養手当
父母の離婚・死亡などによって、父 または母と生計が同一でない家庭や、
父又は母に一定の障害がある家庭で、
18 歳になる年の年度末(3 月 31 日)ま での児童（一定の障害がある児童は 20 歳になるまで）を養育している方に支 給されます。ただし、所得制限があり ます。また、手当の支給対象となるの は、申請を受け付けた日の翌月分から となります。手当を受けている方で住 所が変わった場合は、住所変更の届け が必要です。詳しくはお尋ねください。
◎特別児童扶養手当
精神または身体に一定の障害がある 20 歳未満の児童を家庭で養育してい る父母、またはそれに代わる方に支給 されます。ただし、所得制限がありま す。県内の他市町村から転入された方 は証書・印鑑、県外から転入された方 は証書・印鑑・住民票（世帯全員）を 持って、子育て支援課へお越しくださ い。
手当を受けている方の住所が変わっ た場合は、住所変更の届けが必要です。
◎母子・寡婦・父子への貸付け 母子家庭と寡婦家庭および父子家庭 の経済的自立を図るために、修学資金、
就学支度金などの福祉資金の貸し付け をしています。

◎Child allowance
Aiming to provide support for raising children who will play a key role in the future, Child Allowance is paid for those who bring up children before they complete junior high school education. Please take procedure for application in case of move-in Kawagoe City or childbirth Please note that this system may be changed.
◎Child Raising Allowance
Child Raising Allowances are available for the guardians who are raising children younger 18 years old until the end of the fiscal year (March 31) with parent divorce/death or the disability of either father or mother, not living in the same household. (In case of having a certain disability degree, this allowance paid to the child until 20 years old.) However, income restrictions apply. The eligible period for this allowance is the next month from the day of an application acceptance. If the addresses of those receiving this allowance change, please take procedure for address change.

◎Special Child Raising Allowance
Special child raising allowances are available for parents or guardians who are raising children younger than 20 years old who have physical or emotional disabilities.
Income restrictions apply.
Those who move into Kawagoe city from other cities, towns or villages in Saitama Prefecture visit the Child Rearing Support Division with Certificate and Seal. Those who move into Kawagoe from other Prefectures visit the Child Rearing Support Division with Certificate, Seal and Resident Card of all household members.
If the address of those who receive this allowance changes, it is necessary to take procedure for address change.
◎Financial Loans for Mother-and-Child, Father-and-Child and Widow
Financial loans to be used for vocational training or school are available for household with single mothers, fathers and widows.

■保育
◎保育園
保育園は、保護者が仕事を持ってい たり疾病などのため家庭で乳幼児の保 育ができなかったりする場合、「児童福 祉法」に基づき保護者に代わって保育 することを目的とした施設です。市内 には、市立保育園 20園、法人立保育 園 18園があります。
◎家庭保育室
保護者が仕事や疾病などのため、家 庭で保育することができない生後8週 間 以 上 3 歳 未 満 の 乳 幼 児 を 保 育 し ま す。
◎一時的保育
保護者が労働・職業訓練など、また は傷病・災害・事故・出産・看護・介 護・冠婚葬祭・裁判員に選ばれた場合 などで一時的に家庭保育が困難なとき に、一部の保育園で子どもを保育しま す。

■ Childcare
◎Nursery School
Nursery schools are facilities based on Child Welfare Act that provide child care for infants and toddlers of parents who are not able to care for them during the day because of employment or illness. There are 20 municipal nursery schools and 9 private nursery schools in the city.
◎Home Child care
Child care services are available for parents with 8-seeks-old to 3-years old infants and toddlers who are unable to care for their children during the day because of work or illness.
◎Temporary Child Care
Temporary child care services are available at some nursery schools for parents who are unable to care their children at home because of works, training schools, illness, disaster, accident, childbirth, taking care of an ill family member, providing nursing care for an elderly family member or due to a formal events such as a wedding or funeral or a participation as an appointed member of citizen judge.
