Environmental Exposure and Systemic Lupus Erythematosus:

A Systematic Review

Abstract:Systemic lupus erythematosus(SLE)is an autoimmune connective tissue disease. The  etiology of SLE is unknown, however, similar to many autoimmune diseases, it is thought that  environmental factors trigger this disease in genetically predisposed individuals. In this study,  we conducted a systematic review of cohort studies and casecontrol studies which evaluated the  effect of environmental factors, such as smoking, alcohol drinking and hair dye use, on the inci- dence of SLE. We searched the 19932008 PubMed database for original articles which had been  published in English. A total of 32 articles were included in the analysis. The data regarding  the  relationship  between  the  various  environmental  factors  and  the  incidence  of  SLE  were  con- flicting, but suggest that smoking habits, the use of postmenopausal hormones, and a family his- tory  of  SLE  may  be  associated  with  an  increased  risk  of  SLE. On  the  other  hand,  alcohol  drinking  was  reported  to  be  significantly  inversely  associated  with  SLE  in  several  papers.   Be- cause most of studies were carried out in Western countries, the application of these findings to  people in other countries, including Japan, may not be appropriate. Further studies on the in- cidence of SLE are therefore required to conclude the relationship of environmental factors and  SLE.

 要旨:今回我々は,社会医学実習の一環として,根拠に基づく医学の実際を学んだ.対象疾患として全 身性エリテマトーデス(SLE)をとりあげ,発症関連環境要因に関するエビデンスをまとめた.科学論文 検索システムである PubMed を活用し,環境要因と SLE との関連に関する疫学論文を検索した.最新の ものから過去15年までさかのぼって,合計32編の原著論文を収集した.環境要因として,身長や体重と いった身体状態,喫煙や飲酒等の生活習慣,既往歴,家族歴,アレルギー,服薬状況など,様々な要因に ついて検討されていた.しかしながら,統計学的に有意な関連を示した要因は少なく,関連がみられない と報告された要因が多かった.喫煙や飲酒については比較的多くの論文で検討されており,喫煙との間に
