STRONG LEADERSHIP BY MIDWIVES FOR WOMEN'S HEALTH

Leaders think long-term. They think about the group they are leading in the larger
environmental context and recognize global trends. Leaders reach and influence people beyond their field. They have vision, values, stamina, and motivation as well as political skills to influence national health policy. And leaders think in terms of systems renewal for new, changed realities. In the presentation, these distinguishing features of leadership are cast against education for empowered leaders; leadership by midwives for disease prevention and health promotion; and leadership in midwifery for improved health system performance.
Suggestions for education to prepare midwives for leadership for women's health include: a) requiring that licensed midwives be trained at the master's level; b) ensuring that genetics is a sizable component of midwifery education; c) preparing midwives to lead in health policy arenas with high government officials; and d) mandating evidence of lifelong learning by practicing midwives to ensure their competence and patient safety, but without overburdening them.
Japan's health ministry is especially concerned about the health implications of tobacco usage, violence against and abuse of women and children, and adolescent pregnancy. Therefore, the disease prevention and health promotion aspects of each of these health issues for midwifery, as well as several health policy implications are discussed in terms of the leadership that is and can be provided by midwives in communities, prefectures, and nationally. Several international comparisons are made.
National and local leadership by midwives for improved health system performance concludes the discussion. Measurement of system quality and performance and cost effectiveness of medical and midwifery activities are addressed. Questions that can be contemplated for quality improvement, cross system comparisons, and system renewal are presented. The role of leaders in building sustainable systems for women's health concludes the presentation.

沖縄県立看護大学教授、イ リノイ大学シカゴ校名誉教授 リー ダー とな る者 は長 期 的 な視 点 で考 え る こ とが 求 め られ ます 。 リー ドして ゆ く集 団 の 置 かれ た環 境 とい う大 きな 文 脈 の 中 で 考 え な が ら、 グ ロー バ ル な 動 向 を把 握 します 。 ま た、 専 門分 野外 の人 々 に も接 触 し影響 を与 え ます 。将 来 へ の展 望 、価 値 観 、スタミナ 、意 欲 を持 つ と 同 時 に 、 国 の保健 医 療 政 策 に働 きか け る政 治 的 手 腕 も持 つ 必 要 が あ ります 。 さ らに 、現 実 を 変 えて い くた め に シス テ ムの 刷新 に つ い て も考 え ます 。 この よ うな リー ダー シ ップ の特 徴 に つ い て 、今 回 の 講 演 で は 、1)リ ー ダー の養 成 の た め の教 育 、2)疾 病 予 防 な らび に 健康 増進 にお け る助 産 師 の リー ダ ー シ ップ 、3)保 健 医 療 シス テ ム のパ フ ォー マ ン ス改 善 に お け る助 産 師 の リー ダー シ ップ 、 とい う面 か ら取 り上 げ て ゆ きま す 。 助 産 師 が女 性 の健 康 のた め に リー ダー シ ップ を発 揮 で き る よ うに教 育す る こ とに 関 して は、 次 の よ うな 提言 を お こな い ます 。a)助 産 師免 許 の要 件 と して修 士 レベ ル で の教 育 を課 す こ と、b)助 産 教 育 の 一 環 と して 相応 の遺 伝 学 を確 保 す る こ と、c)政 府 ・自治 体 の 幹 部 と とも に 保 健 医 療 政 策 の 分 野 で リー ダ ー シ ッ プ を発 揮 で き る よ うな 助 産 師 を 育 成 す る こ と、 d)助 産 師 の能 力 と患 者 の安 全 を確 保 す るた め 、助 産 師 に対 し、継 続 教 育 を受 け てい る とい う エ ビデ ンス を義 務 づ け る一 方 で 、 そ れ が負 担 に な りす ぎ な い よ うに す る こ と。 日本 の厚 生労 働 省 が特 に懸 念 を示 して い る事 項 と して 、喫 煙 、女 性や 子 ど もに対 す る暴 力 ・ 虐 待 、若 年 妊 娠 の それ ぞれ が 健 康 に 及 ぼ す影 響 が あ げ られ ます。 した が っ て 、 この よ うな健 康 上 の 各課 題 にお け る疾 病 予 防 や 健 康 増 進 の側 面 につ い て、 保 健 政 策 との 関連 事 項 も い くつ か 取 り上 げ な が ら、地 域 、全 県 、 全 国 で 助 産 師 が どの よ うな リー ダ ー シ ップ を発 揮 して い る の か、 ま た発 揮 して い け るのか を論 じる こ とに します。 併せ て、 国際 的 な比較 もお こない ます。 最 後 に 、助 産 師 が地 域 や 全 国 で 発揮 す る リー ダー シ ップの うち、保 健 医療 シ ス テ ム のパ フ ォ ー マ ン ス を改 善 す る面 につ い て 論 じま す。 こ こで は 、 シ ステ ム の 質 ・パ フォ ー マ ン ス を測 定 した り、 医療 ・助 産 行 為 の費 用 効 果 を測 定 した りす る こ とにつ い て 取 り上 げ ま す。 ま た 、 質 の 改善 、 シ ス テ ム の ク ロス 比 較 、 シ ス テ ム の一 新 とい った 面 で想 定 され うる質 疑 事 項 を示 します 。 さ らに 、女 性 の健 康 のた め に持続 可能 な シ ス テ ム を構 築 す る こ とに お い て 、 リー ダ ー は どの よ うな役 割 を果 た す べ き か に つ い て 述 べ 、本 講演 を締 め くく りた い と思い ます。 (翻訳 與 那 嶺 敦) 日本 助 産 学 会 誌 第16巻 第3号(2003.3) 37
