科 学 的 根 拠 に 基 づ い た 構 音 障 害 治 療 を 目 指 す 標 準 的 評 価 指 針 の 策 定
藤 原 百 合＊ , 1 )、 山 本 一 郎 2 ）、 氏 平 明 3 ）、 緒 方 祐 子 4 ）、 中 村 哲 也 1 ） 1 )聖 隷 ク リ ス ト フ ァ ー 大 学 、2 )山 本 歯 科 医 院 、3 )豊 橋 技 術 科 学 大 学 、4 ）鹿 児 島 大 学 構 音 障 害 の あ る 人 は コ ミ ュ ニ ケ ー シ ョ ン 活 動 に 支 障 を き た し 、 社 会 的 参 加 が 制 約 さ れ る こ と も あ る 。 治 療 に よ る 改 善 が 期 待 さ れ る が 、 こ れ ま で の 構 音 障 害 に 対 す る 治 療 方 法 は 多 分 に 経 験 か ら 生 み 出 さ れ た も の で あ り 、 科 学 的 根 拠 に 基 づ い た も の は 少 な か っ た 。 そ れ は 一 施 設 の 症 例 数 に 限 度 が あ り 症 例 対 照 研 究 の 域 を 出 な か っ た か ら で あ る 。 EBP を 目 指 す に は 多 施 設 間 、 多 言 語 間 で の 共 同 研 究 が 望 ま れ る 。 そ の 際 考 慮 す べ き 評 価 基 準 や デ ー タ 収 集 方 法 の 標 準 化 に 向 け て 、 国 内 外 に お け る 構 音 障 害 の 分 類 と 名 称 に 関 す る コ ン セ ン サ ス を 得 る こ と を 目 標 と し た 。 課 題 １ ： 第 ２ 回 EPG シ ン ポ ジ ウ ム （ 2012 年 5 月 27 日 、 大 阪 ） テ ー マ ： EPG を 研 究 や 臨 床 に 如 何 に 活 用 す る かシ ン ポ ジ ス ト：Fiona Gibbon 教 授（ University Cork, Ireland）、Alan Wrench 博 士（ Queen Margaret University, UK）、 山 本 一 郎 、 藤 原 百 合

課 題 ２ ： the 14th Meeting of the International Clinical Phonetics and Linguistics Association（ June 27-30, 2012, University Cork, Ireland ）
Title: Reconsidering Japanese terminology of consonant production errors, using EPG patterns in relation to perceptual evaluation
Purpose:・ Investigate some controversies on the precise criteria and terminolog y of Japanese articulation error categories
・ Identify an appropriate terminology in global terms Materials and methods:
・ Thirty abnormal patterns were extracted from EPG clinical data.
・Participants: 6 speech therapists who have more than 10 years of cl inical experience with cleft palate clients
・ Assessment: using research data ① speech sounds alone
using research data ② speech sounds and EPG movie Result：
① listening alone： 8/30（ 27％ ） of concordance
② listening and viewing EPG pattern： 18/30（ 60％ ） of concordance Conclusion:
The rate of concordance was higher when EPG movie was introduced in addition to speech sounds. The predominant EPG pattern of “ palatalized misarticulation” was “ retraction” , which corresponds to “ posterior CTC’ s” or “ abnormal backing of oral targets, but place remains oral” . It is suggested that the Japanese term of “ palatalized misarticulation” is inappropriate and might cause misunderstanding among professionals of other countries.
