アルギニンは免疫機能改善や蛋白合成の亢進、創傷治癒を促進する役割があり、微小循環調整 11
に大切な一酸化窒素(NO)の基質である。一方で、過剰な一酸化窒素の産生により、末梢血管の 12
過度な拡張や循環動態への悪影響が危惧されている。Galban らは敗血症患者 176 名を対象にア 13
ルギニンを強化した栄養剤群がコントロール栄養剤群に比べて死亡率が有意に低下したと報告し 14
た(19% vs 32%)1)。一方、Dent らは敗血症患者 170 名を対象にアルギニンを強化した栄養剤群は 15
コントロール栄養剤群に比べて死亡率が有意に上昇したと報告した(23% vs 10%)2)。Kieft らは ICU 16
患者 597 名に対し、アルギニンを強化した栄養剤群とコントロール群を比較し、死亡率、感染症発 17
生率、ICU 滞在日数などに差が無かったことを報告した3)。これらの文献を含む 24 文献をメタ解析 18
すると、集中治療患者に対するアルギニン強化栄養剤の使用は死亡率、感染症発生率に影響し 19
なかった 1-25)。外傷や熱傷を除く、敗血症、重症患者へのアルギニン投与のサブグループ解析で
20
は死亡率、感染症発生率に差を認めなかった 1-13)。外傷患者、熱傷を含んだ重症患者へのアル 21
ギニン投与のサブグループ解析でも死亡率、感染症発生率に差を認めなかった14-24)。 22
以上より、敗血症、重症患者へのアルギニン強化栄養剤の効果の評価は定まっておらず、病態 23
を悪化させる報告が複数あることから 2,6,10)、アルギニンを強化した栄養剤は重症患者には使用し 24
ないことを弱く推奨した。

グルタミンは抗酸化反応、免疫機能、heat shock protein 産生などに関与し、様々な有効性を発 12
揮する条件付き必須アミノ酸である。消化管においては腸上皮細胞の栄養となり、腸管の 13
integrity を維持する。

集中治療患者に対するグルタミン強化経腸栄養の有効性について、Garrel ら1)は、グルタミン強 15
化により有意に死亡率が低下した(10% vs 50%、p < 0.05)と報告しているが、この報告は熱傷患者 16
を対象としたものであった。しかし、その他の報告ではグルタミン強化栄養の死亡率に関する有効 17
性は認められておらず、集中治療患者を対象とした 9 文献1-9)によるメタ解析の結果では、グルタミ 18
ン強化栄養剤投与の有無によって死亡率に差はみられなかった 10)。感染性合併症に関しては 3 19
文献の報告があり、Houdijk ら 3)は、外傷患者においてグルタミン強化栄養投与により肺炎、菌血 20
症、敗血症の発生が有意に低下した(p < 0.02)と、また、Zhou ら4)は、熱傷患者において創感染の 21
発生率が有意に低下した(p < 0.05)と報告している。しかし、混合型 ICU 患者を対象とした Hall ら 22
2)の報告では、グルタミン強化栄養の感染症発生に対する有効性は示されなかった。在院日数に
23
関する記載のあった 5 文献のメタ解析では、グルタミン強化栄養の投与により有意に在院日数が 24
短縮した(p = 0.002)が、これらは外傷3,5,6と熱傷4,7を対象とした検討であった10)。 25
Heyland らはグルタミン投与(経静脈的投与 0.35g/理想体重 kg/日と経腸投与 30g/日)とセレ 26
ン・抗酸化物質投与による大規模な臨床研究(REDOXS Study)を行い、衝撃的な結果をもたらし 27
た11)。対象患者は 2 つ以上の臓器障害のある患者 1223 名であり、グルタミンを投与した患者群と 28
投与していない患者群を比較すると、28 日死亡率(32.4% vs 27.2%,p<0.05)、病院内死亡率(37.2%

vs 31.0%, p<0.05)、6 カ月後死亡率(43.7% vs 37.2%, p<0.05)と有意に上昇した。この研究では本邦 30
では使用できない経静脈投与のグルタミンを併用して経腸栄養投与と合わせて 50g前後の大量 31
グルタミン投与を行っている点は注意する。但し、死亡した群ではグルタミンを投与する前からグ 32
ルタミン血中濃度が上昇している症例があり、ショック、臓器障害を呈する重症患者には外傷、熱 33

傷含めてグルタミン投与を控えることを強く推奨する。

van Zanten らは人工呼吸を用いた集中治療患者 301 名を対象に、高たんぱく(カゼイン、グルタ 2
ミン、麦含有)、脂質(EPA,DHA,MCT)、抗酸化物質(Vitamin C, Vitamine E, セレン、亜鉛)を強化 3
した高たんぱく免疫修飾栄養剤と、高たんぱく(カゼイン)栄養剤投与による比較を行った12)。高た 4
んぱく免疫修飾栄養投与群はグルタミン量は 30g/1920kcal、タンパク成分 24%、脂質成分は 5
46%、高たんぱく栄養投与群はグルタミン量は 9g/1920kcal、タンパク成分 24%、脂質成分 27%

の投与である。外傷、外科術後の集中治療患者に対しては死亡率、感染率など差を認めなかっ 7
た。内科的な集中治療患者に対しては、感染率、28 日死亡率など差を認めなかったが、6 か月後 8
死亡率(54% vs 35%, p<0.05)では有意に上昇した。グルタミンのみを強化した研究ではないが、長 9
期死亡率に影響を与えた研究であることは注意を要する。参考として本邦で使用されるグルタミ 10
ン・ファイバー・オリゴ糖は一包でグルタミン 3gを含有していることは知っておくべきであるが、この 11
栄養剤を使用した集中治療患者へのRCTはないのが現状である。

したがって、グルタミンを強化した経腸栄養剤の投与は、熱傷や外傷患者では死亡率の改善、
13
感染性合併症発生率の低下、在院日数短縮などの点から考慮すべきといえる。グルタミンを経腸 14
的に用いる際、これまでの報告から推奨投与量は 0.3~0.5g/kg/day と考えられる。しかし、その他 15
の集中治療患者に対してグルタミン強化栄養剤をルーチンに投与することは有効性が確認されて 16
いないため推奨するには至らない。また、ショック、臓器障害を呈する重症患者には外傷、熱傷含 17
めてグルタミン投与を控えることを強く推奨する。

なお、Signet Trial13)におけるグルタミン投与は静脈投与であり、本邦ではグルタミンの静脈投与 19
製剤が市場に存在しないため、本項では議論の対象から外すこととした。
