Letter of Pledge
I, as an individual wishing to enter Japan (hereinafter called “an entrant”) hereby pledge to meet and comply with the following requirements in good faith in the event that President of Gifu University (hereinafter called, “the President”) makes an online application to enter Japan on behalf of an entrant.
1. Requirements to be met by an entrant A) Pre-departure Preparations
i. An entrant is required to undergo COVID-19 testing approved by the Ministry of Health, Labour and Welfare (MHLW) of Japan within 72 hours before departing his/her native country, and obtain a “negative” test result certificate from a local medical institution. An entrant is required to show and submit the said certificate (or copy thereof) to the airport quarantine officer and immigration officer in Japan. If an entrant are not able to show and submit the said certificate (or copy thereof) to the immigration officer, an entrant must understand that he/she may become ineligible to enter Japan in accordance with the Emigration and Immigration Management and Refugee Recognition Law.
ii. An entrant must purchase either a private medical insurance policy (including a travel insurance which covers medical expenses during his/her stay in Japan) or a public health insurance policy before entering Japan.
iii. Before entering Japan, an entrant is asked to obtain a smartphone and download the following apps: 1) MySOS (Monitoring App for Overseas Entrants) 2) Google Maps (Location Information App), and 3) COCOA (COVID-19 Contact-Confirming Application). An entrant is also asked to register necessary information (e.g. uploading negative test result certificate) to MySOS before entering Japan. In case an entrant is not able to obtain a smartphone before leaving his/her country, an entrant must rentw a smartphone and download the abovementioned apps at the
B) Self-Isolation at Home after Entering Japan
i. An entrant who is from “Group Red” country/territory designated by the Government of Japan in response to COVID-19, and has been unvaccinated for third booster shot, is required to stay at the accommodation arranged by the quarantine office for three days from the following day after his/her entry to Japan. If PCR etc. test result is negative, an entrant can leave the accommodation and move to his/her home whose address is written in this
“Letter of Pledge.” If an entrant tested positive, he/she must follow the instructions of MHLW.
ii. An entrant who is from “Group Red” country/territory designated by the Government of Japan in response to COVID-19, and has been vaccinated for third booster shot, or an entrant who is from “Group Yellow”
country/territory, and has been unvaccinated for third booster shot, is required to stay at the accommodation arranged by an entrant. If an entrant tested negative for PCR etc. testing he/she underwent voluntarily from the third day onward after the following day of his/her entry to Japan, his/her self-isolation period may be shortened. The “accommodation” here can be defined as hotel, apartment building or his/her own home with no possibility of immediate contact with others.
iii. An entrant who is from “Group Yellow” country/territory designated by the Government of Japan in response to COVID-19, and has been vaccinated for third booster shot, or an entrant who is from “Group Blue” country/territory is not required to self-isolate at home after entering Japan, but is asked to promptly move to his/her home in Japan.

iv. If there are any changes made for mandatory self-isolation period by the country/territory designated by the Government of Japan in response to COVID-19, an entrant must follow the instructions of the University or MHLW.
v. Gifu University (hereinafter called, “the University”) does not allow an entrant to stay at International House (hereinafter called, “the House”) at the University as “accommodation” mentioned at ii above.
C) Use of Public Transportation after Entering Japan i. Limit to use public transportation system
An entrant of B ii) is allowed to use public transportation only when he/she travels to the designated accommodation for self-isolation within 24 hours after a negative test result.
ii. Please observe the following infection prevention protocol in time of using public transportation:
1) Wear a face mask 2) Use hand sanitizer
3) Avoid the “Three Cs (closed spaces, crowded places and close-contact settings)
D) Vaccine Certificate
Please confirm the following before an entrant shows his/her COVID-19 vaccine certificate to the airport quarantine officer:
*An entrant also needs to submit the copy of his/her vaccine certificate to the University.
a. Filled in all information from 1 – 5 either in Japanese or English:

Note 1) As long as Japanese or English translation is attached and whose content is verified, a certificate is considered to be valid.
b. Certificate that officially certifies that an entrant had received the vaccines indicated at “Table 1” twice, and received the vaccines indicated at “Table 2”
as third booster shot.
*In case an entrant received “Janssen COVID-19 Vaccine” (Janssen) once, he/she is considered to have received vaccines twice already. The same shall
apply hereinafter.

E) Monitoring Health
a. An entrant needs to take body temperature and log his/her physical conditions every day for 10 DAYS from the following day after entering Japan even after his/her mandatory self-isolation period is over. Physical conditions to log include high fever, breathing abnormalities, fatigues, etc. all of which could represent typical symptoms of COVID-19.
b. If any abnormalities are confirmed, an entrant needs to promptly contact the staff of his /her host faculty/school/center, etc. (hereinafter called “host faculty”) at Gifu University and go to the hospital. The staff will contact the chief at International General Affairs Section, International Engagement Division, Student Affairs Department.
c. An entrant needs to follow every instruction from MHLW.
F) From Country/Territory where dominance of variant other than Omicron is confirmed
An entrant who is from the country/territory where dominance of variant other than Omicron is confirmed, needs to self-isolate at home/designated accommodation for seven days from the following day after entering Japan.
G) Purposes of Entering Japan
The revision of the Government of Japan’s border control policy effective on March 1, 2022 requires the University (host institution) to take full responsibility for actions of an entrant. Therefore, individual entrants must understand their purposes of entering Japan, and are always asked to act responsibly.
H) Life in Japan
i. If an entrant wishes to stay in the House after his/her self-isolation period, he/she needs to have applied for residence and gotten permission from the University.
ii. If an entrant plans to seek off-campus accommodation, he/she must ensure that he/she can find a decent accommodation in Japan.
I) Other
a. If an entrant should violate any clauses of this Letter of Pledge addressed to the President or should violate any clauses of the “Written Pledge” addressed to the MHLW Minister or the Minister of Ministry of Justice submitted to the quarantine officer upon entering Japan (including falsified information on the pledges), an entrant may become ineligible for online application to enter Japan from the perspective of COVID-19 infection prevention.
b. An entrant must observe every rule and regulation in Japan including border control law, quarantine law, Act on the Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases, and border control relevant laws such as Immigration Control and Refugee Recognition Act.
