Dietary Intake, Fatty Acid Profile in Plasma and Neutrophil Phospholipids, and Serum Antioxidant Levels

in Patients with Crohn's Disease

T o elucidate the efficacy of dietary therapy for Crohn's disease, we analyzed dietary intake, fatty acid composition of phospholipids in plasma and neutrophils, serum fat-soluble vitamin levels, and oxygen radical absorbance capacity in twenty Crohn's disease patients (15 males and 5 females, 11 with ileitis and 9 with ileocolitis, 30.6k8.2 year), who were treated at the Department of Gastroenterology of Okayama University Hospital. Total fat intake, fat energy ratio and linoleic acid intake were significantly lower, while protein and carbohydrate intakes were significantly higher, in the patients than in age and sex-matched controls. In the neutrophil phospholipids of Crohn's disease patients, significantly higher levels of total n-6 polyunsaturated fatty acid and lower levels of docosahexaenoic acid were observed. The concentrations of serum retinol and 8-carotene but not a-tocopherol were significantly lower and serum oxygen radical absorbance capacity was also lower than in the controls. Significant correlations between serum oxygen radical absorbance capacity and zinc ( ~ 0 . 7 9 7 , p<0.001) concentrations were observed in the Crohn's disease patients.

A diet restricting the intake of n-6 polyunsaturated fatty acid and antioxidative trace minerals and vitamins may be recommended for the nutritional management of Crohn's disease patients.

Introduction

Nutrition may play an important role in the patho- genesis and treatment of inflammatory bowel diseases (IBD)

[I, 2, 31.

Several studies suggest an associa- tion between dietary factors and the onset of Crohn's disease (CD),

however, few have examined the relationship between dietary intake and relapse of

may be implicated in the relapse of CD. Although the pathogenesis of inflam- matory bowel diseases is not fully understood, many suspect that diet and various dietary factors may play a modulating role in the disease process

because long chain polyunsaturated fatty acids (PUFA) are the precursors of eicosanoids, which participate in the regulation of immunological and inflammatory responses. The balance between oxi- dant and antioxidant systems is suggested to be

important in the pathogenesis and maintenance of tis- sue injury in CD. Reduced plasma antioxidant con- centrations and increased oxidative DNA damage in patients with Crohn's disease and Ulcerative Colitis (UC) were reported

[81.

Adequate nutritional management of CD as a moderator of intestinal inflammation may be required for remission of the disease. In this study, there- fore, to elucidate the efficacy of dietary therapy for CD, we analyzed the qualitative and quantitative dietary intake, fatty acid composition of phospholip- ids in plasma and neutrophils, serum levels of fat- soluble vitamins, retinol, &carotene and a-tocoph- erol, and Oxygen Radical Absorbance Capacity (ORAC), and evaluated the effects of nutritional fac- tors on the clinical parameters.

Methods

Subjects

Twenty CD patients were examined in the present study. They were all outpatients treated at the Department of Gastroenterology of Okayama Univer- sity Hospital. Of the patients,

had ileitis and 9 had ileocolitis. The diagnosis of CD was based on accepted clinical, radiographic, endoscopic, and his- tologic criteria [9]. All patients were in remission.

Nineteen patients were medicated with 5-aminosali- cylic acid (5-ASA), 2 with azathioprin and

with an elemental diet a t the time of the study. Five patients consumed regular diets. Control subjects were age and sex-matched healthy volunteers, 12 males and 5 females (29.4 k

year).

This experiment was performed in accordance with the principles of the Declaration of Helsinki of the World Medical Association, and informed con- sent was obtained from all the subjects. This study was approved by our institutional review board.

Estimation of dietary intake

Dietary intake was assessed using a food-fre- quency questionnaire. Energy, major nutrients and fatty acid consumption were calculated using a method reported by Suzuki et a1 [lo].

Fatty acid analysis

Fasted blood was drawn into tubes containing disodium EDTA. Plasma was separated after the centrifugation of blood samples at 1 , 6 0 0 ~ g for

min at 4°C and stored at -80°C until assayed. To

obtain neutrophils, a 3.5-ml sample of undiluted blood was layered onto 3ml of Mono Poly Resolving Medium (Dainippon Pharmaceuticals, Osaka, Japan) and centrifuged at 400x g for 20min at 4OC. The neutrophil layer was drawn off into another tube.

The neutrophils were washed

times with ice-cold saline, and stored at -80 'C prior to use.

Total lipid was extracted from plasma and neutro- phils suspended in 0.5ml of saline according to the method of Bligh and Dyer [ I l l . Total phospholipid was separated by one-dimensional thin-layer chroma- tography using silica gel plates (Silica Gel 60, Merck, Darmstadt, Germany) and a solvent system of petroleum ether/ethyl ether/acetic acid (80: 20: 1, v/v/v). The spots corresponding to phospholipid were scraped from the plates and transmethylated for 2 h a t

with 2ml of acetyl-chloride methanol (5: 50, v/v). The fatty acid composition of total phospholipid was analyzed according to a method described previously [121.

Analysis of fat-soluble vitamins in plasma

Retinol, a-tocopherol and carotenoids were extracted from plasma and analyzed using the method reported by Milne e t al. 1131. Briefly,

ml of meth- anol was added to each sample (0.2ml of plasma plus 0.8ml of distilled water) and extracted twice with 3ml of dichloromethane and 6ml of n-hexane. After shaking and centrifugation (3,000 rpm, 10 OC

,

15min), the upper layer was collected and evapo- rated under reduced pressure. The residue was dis- solved in 50 pl of acetonitrile-dichloromethane-metha- no1 (70: 20: 10, v/v/v), and a 50-pl aliquot was injected into the HPLC system. The system was composed of a reverse-phase column (YMC s-5 120A ODs,

x 150mm, Yamamura Chemical Co., Kyoto, Japan) and integrator (CR-4A, Shimadzu Co., Kyoto, Japan). The injected sample was eluted iso- cratically with acetonitrile-dichloromethane-methanol (70: 20 : 10, v/v/v), at a flow rate of

ml/min, with detectors set at 290nm, 350nm and 450nm. Retinol, a-tocopherol, and P-carotene were quantified by determining the peak areas calibrated against known amounts of standards.

Oxygen radical absorbance capacity (ORA C) assay

Serum total antioxidant capacity was measured using the manual version of the oxygen radical absor- bance capacity (ORAC) assay as described by Cao et

al. [141. One run of the ORAC assay comprised one blank, one standard and

serum samples. Phosphate buffer (1.75ml) and R-phycoerythrin (R-PE, 3.73mg/

1; 100fi1, Sigma, S t Louis, MO, US) were added to each of

fluorimeter cuvettes. The cuvettes were preincubated for 15min at 37°C. A volume of 100 fil of buffer (blank), 20 ,uM Trolox (standard, Aldrich Chemical Co., Milwaukee, WI) o r diluted serum (sample) was then added. The reaction was started by the addition of 320 mM 2,2-azobis (2-amidino-pro- pane) dihydrochloride (AAPH, Wako Pure Chemi- cals Co., Ltd., Osaka, Japan) solution. The fluores- cence was measured using the JASCO FP-6300 fluorescence spectrophotometer (emission wavelength 575 nm, excitation 495 nm). The cuvettes were incu- bated at 37°C during measurements and the fluores- cence was recorded every 2min it had diminished to less than

of the initial value. The final result was determined by calculating the difference of area under the R-PE decay curve between the blank and a sample, and expressed using Trolox equivalents.

Evaluation of serum TNF-a was performed using commercially available enzyme-amplified sensitive immunoassays ( I M M U N O T E C H , Marseille, France). The minimal detectable concentration of TNF-cr was

pg/ml.

Statistical analysis

All statistical calculations were performed using

SPSS

software. Results are expressed as means

+

standard deviations. The significance of differences was determined with an unpaired two-tailed t-test and a Mann-Whitney U-test. Correlation coefficients were calculated by Spearman's rank-correlation anal- ysis when appropriate. Two-sided p values less than 0.05 were considered significant.

Result

The body mass index (BMI) and the findings of clinical parameters in the controls and the patients with CD are shown in Table 1. The BMI was within the normal range for 19 patients. The BMI of

patient was low (14.1). Although white blood cell (WBC) counts were also within the normal range, the lymphocyte ratio and lymphocytes were signifi- cantly lower in the patients than the controls. Serum total protein, albumin and hemoglobin concentrations

were also significantly lower in the CD patients. The serum sialic acid level was significantly higher than in the controls. There were no significant differences in serum, TNF-a (21.7 -t 14.9, 20.2 k 10.6pg/ml), and zinc (76.1 k 15.6, 82.5 k 14.6mg/dl) levels between the CD patients and controls. The serum copper (126.0 -t 25.5, 90.2 -t 26.1 mg/dl, respec- tively) levels were significantly higher (P<0.001) than in the controls.

Table 2 shows intakes of energy and major nutri- ents. In the CD patients, fat intake and the fat energy ratio were significantly lower, while protein and carbohydrate intakes were significantly higher.

significantly lower in the patients. However, no sig- nificant difference was observed in the serum a -tocopherol level between the controls and the CD patients. Significant correlations between the serum ORAC value and zinc (r=0.797, p<0.001) concen- trations were observed in the CD patients.

Discussion

In the CD patients studied, serum total protein and albumin levels were significantly lower than lev- els in controls, however, they remained within the normal range and there were no patients classified as malnourished. The CD patients were instructed to consume a diet rich in energy and protein, but restricted in fat and dietary fiber, and they followed their dietitian's advice well. Fifteen of 20 CD patients were nourished with an elemental diet, hav- ing high energy and containing high amino acid lev- els, a nitrogen source, vitamins and minerals and low in fat and dietary fiber.

Arachidonic acid is generally not abundant in nat- ural foods and is mostly synthesized from linoleic acid by desaturation and chain elongation in the liver. Linoleic acid intake was lower in the CD patients than the controls, and as a result, linoleic acid and arachidonic acid molar percentages in the plasma phospholipids of the patients were reduced.

Contrary to the plasma phospholipids, significant increases of the levels of n-6 P U F A and arachidonic acid in neutrophil phospholipids were observed in the CD patients. The patients were medicated with 5-ASA and/or azathioprin. 5-ASA provide anti- inflammatory and immuno-suppressing effects by inhibiting the synthesis of eicosanoid from arachi- donic acid by the cyclooxygenase and lipoxygenase pathways [15, 161. Therefore, inhibition of the metabolism of arachidonic acid may induce such an accumulation of arachidonic acid in neutrophil phos- pholipids and this may be followed by an increase in linoleic acid.

An increase in the arachidonic acid concentration of phospholipids may be accompanied by a rise in phospholipase A2 activity within the intestinal mucosa C171. Leukotriene (LT) B4 is a prominent product of arachidonic acid metabolism in neutro- phils, and a chemotactic factor that facilitates the accumulation of neutrophils with the consequent pro-

duction of reactive oxygen species causing damage from oxidative stress [MI. Eicosapentaenoic acid (EPA) competitively inhibits the cyclooxygenase and lipoxygenase pathways and reduces the production of LTB4 [19], which demonstrated a substantial reduc- tion in clinical relapse rate among patients with

in clinical [20]. An n-3 fatty acid-rich diet prevented an early response to elevated levels of interleukin (1L)-6 compared with an n-6 fatty acid-rich diet in trinitrobenzene sulfonic acid-induced enteritis [21I.

A decrease in E P A and increase in arachidonic acid in neutrophils may not beneficial for CD patients.

Further restriction of dietary linoleic acid and sup- plementation of E P A may facilitate the continuance of remission in CD patients.

The present CD patients in this study had higher protein diets than the controls. Shoda e t al.

suggested that a high meat diet rich in sulphur and sulphate may be implicated in relapses of CD. Amino acids containing sulphur are abundant in animal pro- teins but not in soy protein. Soy products such as soy-curd and soy-milk may be recommended as a source of protein for

patients.

The role of relapse rate in patients with quiescent IBD treated with soluble dietary fiber, which is a substrate precursor for the production of butyrate in the colon, was not higher than that of patients given 5-ASA a t the usual maintenance dose [22]. Although the intake of dietary fiber, both soluble and insolu- ble, in the studied CD patients was the same as that in the controls, the total fiber intake was less than that considered adequate for Japanese adults [231.

A therapeutic approach using dietary fiber, espe- cially soluble fibers acting as prebiotics, may also be possible in CD.

Retinol and P-carotene are fat-soluble vita- mins; therefore, the low fat diets may be an explana- tion for the low serum levels of fat-soluble vitamins observed in the present CD patients.

The significant decrease in the serum ORAC value and significant correlations between the serum ORAC value and serum zinc levels observed in the present patients may indicate a deficiency of antioxi- dative trace minerals. Reactive oxygen metabolites such as superoxide and hydroxyl radicals are pro- duced by activated neutrophils and macrophages and are considered to contribute to the tissue damage seen in IBD. Increases in the concentrations of reac-

tive oxygen species have been found in the plasma and mucosa of patients with

CD [241.

Supple- mentation of antioxidant trace minerals and vitamins may be beneficial for

CD

patients.

From the present observations, diets restricting the intake of vegetable oils rich in linoleic acid and providing

EPA

from fish and antioxidative trace min- erals and vitamins may be recommended for the nutritional management of

CD

patients. These recom- mendations, however, should be tested in further examinations.
