4. Required Documents
Copies of the original documents are acceptable. Samples of the required documents and the issuing bodies are printed on the last page of the Japanese version of this notice.
Reasons for Application Documents
1. In receipt of public assistance (with education aid) 2. In receipt of pubic assistance (without education aid) 3. In case of public assistance being
stopped/abolished
No certificates required. Complete and submit the application form to
school. (Regarding 3, it is applicable for the fiscal years 2006 or 2007. It does not
include the abolition due to a change in a household.)
4. In receipt of a child+care allowance (a child allowance and a special child+care allowance are not applicable for this purpose.)
+ a certificate of the child+care allowance [a copy of the page with the monthly amount of the allowance. (the page with seals of the Mayor of Kyoto City and the prefectural governor)] OR + a certificate of safe+ keeping of the above+mentioned certificate. 5. A life style is unstable due to financial
difficulties and parents cannot easily afford to send their children to school. (A total income of a household is below the criterion explained in 5 below)
For each member of a household, one document to confirm the annual income for 2006 is required. (Details of such
documents are on the back of the application form) However, the proof of income is unnecessary for the following; + Those entitled to a spouse
allowance/deduction for dependents (e.g. students or house wife who fall under such allowances in the income+proof documents. Please refer to the samples.)
6. A household has other specific
reasons. Examples are where a household is taking an emergency shelter due to unforeseen disasters such as fire,
earthquakes, water disasters, violence, etc.
Please consult the school. In some cases, no certificates will be necessary.
