(ファロー四徴症術後遠隔期における、脳性利尿ペプチド上昇の要因とアンギオテンシン変 換酵素阻害薬の効果についての検討)

TOF術後では右室容量負荷(RVVO)等の問題が予後不良因子となるが、外科的介入の適応等 には議論があり、また内科的治療の報告も少ない。今回、内科的治療法の可能性を探る目 的で、TOF術後症例におけるBNPと血行動態指標の関係(BNP study)、ACEIの薬剤量とBNPの 関連(ACEI study)を検討した。BNP studyの31例では、右室拡張末期(RVED)の容積、圧とBNP の相関を認めた。ACEI studyの11例では、体重あたりのACEIが最大時のBNPは最少時より低 く、ACEI増量後ではBNPの低下を認めた。結果、TOF術後では、BNPがRVEDの負荷に相関する 事、またACEIがBNPを低下させる可能性が示唆された。

非公開発表では、副査の三輪聡一教授より、一般なBNPの上昇要因とACEIの血中濃度測定 について質問があった。副査の西村正治教授より小児のBNP測定および右室負荷以外のBNP 上昇要因の質問と、後方視研究へのコメントがあった。副査の筒井裕之教授よりBNPとRVED 圧の関係について、およびACEI studyでの新たな検討項目を指摘された。副査の有賀正教 授から小児例へのACEI投与、ACEIのRVVO以外への効果に関しての質問があった。主査の丸 藤哲教授より右室収縮期圧とBNPの関係、またACEIの効果がある例とない例の相違点の質問 があった。いずれの質問に対しても申請者は妥当な回答をした。

この論文は修正が必要ながらも、TOF術後におけるBNP上昇の要因およびACEI治療の有効 性を示唆した点で高く評価され、今後症例の蓄積からCHD術後における内科的治療の新たな 知見が得られる事も期待される。
