Evidence of remote cognitive behavioral therapy
•
Computerized Cognitive Behavioral Therapy (cCBT)は、DSM-5の不安障害を対象とした場合、wait-listより有 意に効果的(standardized mean difference [SMD] = 0.92 [95% CI, 0.83 to 1.02], k = 31, z = 18.8, P < .001)1•
cCBTの有効性は、子どもと大人の両方で、対面CBTとの有意な差がなく(SMD = 0.05 [95% CI, ー0.09 to 0.19], k = 15, z = 0.7, P = .46)1、見かけ上は同等に有効•
Guided Internet-based Cognitive Behavioral Therapy (gICBT)は、医学的及び身体的疾患のある患者を対象 に、対面CBTと直接比較されると、効果量には有意な差がない(Hedges g = 0.05, 95% CI, ー0.09 to 0.20, P = 0.45)2•

表情や声色、イントネーションなどの非言語情報をリアルタイムでやり取りできるため、おそらく最も対面 CBTに近い遠隔支援アプローチ•
利用者の満足度は概して高く3、日本人を対象とした臨床研究では、86%(n = 25/29)が 満足と回答5•
一部の技法は、対面に比べて実施しにくいかもしれない 例:階段を登る・ランニングする等の内部感覚曝露はできない•

PRSMA fro systematic review protocols (PRISMA-P)の優生レポート項目を使用して、 2020年3月31日までに、Cochrane Central Register of Controlled Trials, PubMed, PsychoInfo, Web of Scienceに掲載された論文についてレビューを構成し、7 以前の体系的レビューの参照から、3, 4 検索されたリストを確認•
選択基準:a) ランダム化比較試験(RCT)である, b) ビデオ会議システムでの介入, c) CBTの技術を含む, d) うつ病, 不 安症, 強迫症, またはPTSDが対象•
除外基準:介入の結果を報告していない(レビューや解説等), 査読付き雑誌に掲載されていない, 英語で書かれていな い, 2次解析論文•
