After life expectancy has been extended in many countries, people became to desire live longer with sustained physical and social functions. Among dwelling environments, accessibility to medical care service may influence health outcomes of elderly. The objective of this study was to determine the activities of daily living in relation to accessibility to hospitals and clinics in elderly people.
We performed multivariable regression for the decline of instrumental activities of daily life (IADL) with the risk factors, using information of the elderly people in the Yamanashi Healthy Active Life Expectancy (Y-HALE) cohort. We also estimated hazard ratios of the need for nursing care in the elderly. A total of 129 female (mean age, 73.5) and 136 male (mean age, 73.2) participated in this study.
The analysis represented baseline age and time to hospital was significantly associated with the decline in IADL. Although there were no statistical significances, the association between time to hospital and the decline in IADL were retained in the strata of both sexes.
Old age and time to hospital may significantly decline the IADL of elderly people. Healthcare professionals and city planners may need to consider the accessibility to hospitals to preserve IADL of residing elderly.
