How do you grade the OJT?
Low grade is 3.00Incomplete is “7” or “IP” or “I”
Failing is “5”

Number of punctually (on time) submitted learning journal (due date every Saturday) (advance submission is counted as on time). ANY item LATE submission is low grade, but NOT to EXCEED one week deadline BEFORE the FINAL exam week is failed.
Learning Journal, wrong format, poor write up, vague write up, short write up (including less than 200 words per OJT day) low grade. (with ALLOWED pictures is a plus, but NOT MANDATORY) Copying from OTHER OJT report or Plagiarize report not citing references is failing.
ON TIME submission of ALL OJT requirements (PRE and FINAL), particularly the completely SIGNED waiver (with CCS signature) any item missing or LATE is considered incomplete. Low grade or “7” incomplete.
Submitting waiver with incomplete signature (no CCS signature) is incomplete.
More than 20% OJT absences or under time (left early) is failing. (School policy).
Host Company complains subject to investigation. Pending school policy resolution of the case.
Not officially enrolling the OJT subject, NO GRADE, NO CREDIT.
NOT attending the MANDATORY OJT Orientation. Low grade
NOT sending email, NOT requesting for OJT procedures. NOT reading the School Bulletin for OJT procedures. Ignorance of Institutional and/or SCHOOL Policies EXCUSES ON ONE.
Not following the CHED OJT requirements (for International OJT) failing or NO credit.
Violation or NOT following ANY SAFETY rules is failing. (riding a motorbike to host company, NOT safe)
Changing email (including once) to confuse the OJT coordinator is LOW grade.
NOT responding to the OJT coordinator’s email inquiry. Low grade.
Submitting ANY FAKE/FALSIFIED company certification and/or documents is SCHOOL EXPLUSION (Read student manual). ALL documents are subject to company inquiry/verification/validation.
REMINDER: Incomplete (7,IP,I) grade will become FAILED “5” after the END term of the NEXT quarterm. So complete ALL requirements before IT LAPSED into FAILED “5” next ending quarterm.
NO GRADE WILL BE given to ANY INCOMPLETE requirements. Even if you are a GRADUATING Student. NO Exemptions. (Remember time management is in your control and your fault)
